This essay is composed of five stories written by practicing physicians about their patients. Each clinical story describes a challenging ethical condition–potential abuse of medical power, gravely ill and probably over-treated newborns, iatrogenic narcotic addiction, deceived dying people. Rather than singling out one ethical conflict to resolve or adjudicate, the authors attempt, through literary methods, to grasp the singular experiences of their patients and to act according to the deep structures of their patients' lives. Examining these five stories with simple (...) literary tools–attention to narrative frames, time, plot, and desire–reveals the mechanisms through which acts of writing and reading contribute to clinical clarity and ethical actions. Edmund D. Pellegrino has played a central role in shaping the fields of bioethics and the philosophy of medicine. His writings encompass original explorations of the healing relationship, the need to place humanism in the medical curriculum, the nature of the patient’s good, and the importance of a virtue-based normative ethics for health care. Everything we do relies on causation. We eat and drink because this causes us to stay alive. Courts tell us who causes crimes, criminology tell us what causes people to commit them. D.H. Psychological literacy, the ability to apply psychological knowledge to personal, family, occupational, community and societal challenges, is promoted as the primary outcome of an undergraduate education in psychology. As the concept of psychological literacy becomes increasingly adopted as the core business of undergraduate psychology training courses world-wide, there is urgent need for the construct to be accurately measured so that student and institutional level progress can be assessed and monitored. Key to the measurement of psychological literacy is determining the underlying (...) factor-structure of psychological literacy. In this paper we provide a first approximation of the measurement of psychological literacy by identifying and evaluating self-report measures for psychological literacy. Multi-item and single-item self-report measures of each of the proposed nine dimensions of psychological literacy were completed by two samples (N = 218 and N = 381) of undergraduate psychology students at an Australian university. Single and multi-item measures of each dimension were weakly to moderately correlated. Exploratory and confirmatory factor analyses of multi-item measures indicated a higher order three factor solution best represented the construct of psychological literacy. The three factors were reflective processes, generic graduate attributes, and psychology as a helping profession. Belief in the ultimacy and distinctiveness of ethical principles has been challenged in many ways to-day. The advance of science, especially in the fields of psychology and anthropology, has provided the relativist and the sceptic with many new weapons to put in their armoury; and the positivist has launched a very subtle attack. The present state of society, both in the internal affairs of the peoples of the world and in their inter-relations, has brought many moral principles into contempt. But (...) there have also been notable advances, especially where more self-effacing virtue is concerned. Philosophers have also rebutted the attacks of the sceptic with peculiar incisiveness and vigour; indeed we know better to-day than at any other time just what we must hold if the objectivity of ethics is to be maintained. How the battle is going is by no means easy to determine. But there is one corner of the field, and that the one from which the most powerful attack of all may be delivered, which the upholder of the ultimacy of moral truths has left almost wholly undefended, so lightly does he estimate the danger from that quarter. That is the religious one. Account has been taken of the arguments of the sociologist and the Freudian psychologist, but it has been assumed, with astonishing naïveté, that religion is an ally to be wholly relied upon. But morality and religion have often been in conflict, and they seem to be so as much as ever to-day. For some of the most powerful forces in our religious life, and those which are in some ways most attuned to our needs, seem to be wholly inimical to the moral life. It is with this aspect of the vast problem of morality and religion that we can best concern ourselves in a course with the general title of “Moral and Political Conflicts of Our Time.”. (shrink)

Mellor] Kant's claim that our knowledge of time is transcendental in his sense, while false of time itself, is true of tenses, i.e. of the locations of events and other temporal entities in McTaggart's A series. This fact can easily, and I think only, be explained by taking time itself to be real but tenseless. /// [J. R. Lucas] Mellor's argument from Kant fails. The difficulties in his first Antinomy are due to topological confusions, not the tensed nature (...) of time. Nor are McTaggart' s difficulties due to the tensed nature of time. The ego-centricity of tensed discourse is an essential feature of communication between selves, each of whom refers himself as 'I', and is required for talking about time as well as experience and agency. Arguments based on the Special Theory are misconceived. Some rest on a confused notion of 'topological simultaneity'. In the General Theory a cosmic time is defined, as also in quantum mechanics, where a natural present is defined by a unique hyperplane of collapse into eigen-ness. (shrink)

This book is unique in presenting an interdisciplinary conversation between jurists and logicians. In this celebrated sentence Thucydides gives his opinion more specifically than anywhere in the narrative of Books 6 and 7 on the reasons for the failure of the Sicilian expedition. Unfortunately, however, he expresses his views with some lack of clarity, and this has led to disagreement among modern scholars who have sought to determine precisely what he means. It is also unfortunate that he does not state the evidence upon which he bases his conclusion that the expedition failed because (...) of struggles for political leadership at Athens. The main purpose of this paper is to consider the meaning and implications of this sentence, which can, I believe, be clarified by reference to other passages in which Thucydides uses when debating the causes of past actions. A closely allied problem, that of the relationship between this sentence and the narrative of Books 6 and 7, will also be discussed. A. W. Gomme has maintained that this relationship throws some light upon the date of composition of Books 6 and 7. There is no doubt whatever that the sentence was written at or after the end of the Peloponnesian war , and if there are differences of outlook, even inconsistencies, between it and the narrative which show that Thucydides cannot have written, or at any rate conceived, both at the same time, he almost certainly wrote the narrative before the end of the war.2 In my opinion there is inconsistency on one point only, but here the inconsistency is sufficiently marked to suggest not only that Books 6 and 7 were written considerably earlier than 2. 65. 11, but that they were written within the next few years after the conclusion of the Sicilian campaign. Professor Monro presents an original view of ethics based on empiricism, which leads him to a subjectivist position about moral values. He starts by examining the central problem in moral philosophy: are moral statements objectively true, or are they expressions of preference? The first view conflicts with the empiricist beliefs current in modern thought; the opposing naturalistic theory seems to lead to moral scepticism. After discussing both views, the author presents a detailed defence of the subjectivist position. In the course (...) of his argument he gives a detailed analysis and criticism of the 'universalisability thesis', the theory that moral aspirations differ from others in being applicable to all men, and that it is this that makes them moral. He then offers an alternative account of the nature of moral attitudes. The author illustrates his explanations with straightforward analogies and examples. His clear exposition of the fundamental concepts arising in his argument makes this a book for students as well as for professional moral philosophers. (shrink)

We examined if cancer patients' health-related quality of life (HRQoL) scores on the European Organisation for Research and Treatment of Cancer (EORTC) QLQ-C30 are affected by the specific time point, before or during treatment, at which the questionnaire is completed, and whether this could bias the overall treatment comparison analyses. A 'completion-time window' variable was created on three closed EORTC randomised control trials in lung (non-small cell lung cancer, NSCLC) and colorectal cancer (CRC) to indicate when the QLQ-30 was completed (...) relative to chemotherapy cycle dates, defined as 'before', 'on' and 'after'. HRQoL mean scores were calculated using a linear mixed model. Statistically significant differences (P < 0.05) were observed on 6 and 5 scales for 'on' and 'after' comparisons in the NSCLC and two-group CRC trial, respectively. As for the three-group CRC trial, several statistical differences were observed in the 'before' to 'on' and the 'on' to 'after' comparisons. For all three trials, including the 'completion-time window' variable in the model resulted in a better fit, but no substantial changes in the treatment effects were noted. We showed that considering the exact timing of completion within specified windows resulted in statistical and potentially clinically significant differences, but it did not alter the conclusions of treatment comparison in these studies. This is a study of the nature of time. In it, redeploying an argument first presented by McTaggart, the author argues that although time itself is real, tense is not. He accounts for the appearance of the reality of tense - our sense of the passage of time, and the fact that our experience occurs in the present - by showing how time is indispensable as a condition of action. Time itself is further analysed, and Dr Mellor gives answers to (...) most of the metaphysical questions it provokes, concerning the relation of time to space, the dissection of time, and its relation to change and causation. To his narrative of the Sacred War Diodorus appends an excursus on the fate of the Phocian leaders, describing at some length the adventures of Phalaecus and his mercenaries after their departure from Thermopylae . The object of this excursus, whose substance probably derives from Demophilus, is to illustrate the terrible consequences of temple-robbery, but to modern scholars the story is interesting chiefly for its portrayal of the difficulties and hardships experienced by mercenary commanders. It does not appear to have (...) been noticed that at the same time Diodorus unconsciously throws some light upon the mission of Timoleon to Sicily. (shrink)

This paper conducts a critical examination of the "usual psychopharmacology standards" for clinical research. Four main areas are inspected: What is "it" that is being treated in a clinical drug study? How much is really known concerning the psychological alterations brought about by psychiatric drug treatment? To what extent are sources of bias actually controlled for in "controlled" drug treatment studies? How does the usual "dropout pattern" influence the alleged "clinical findings" of controlled drug treatment studies? The overall conclusion reached (...) is that the "usual standards" cannot produce a realistic picture of either safety or efficacy. Conceptual and methodological reforms are suggested. (shrink)
